Caregiver burnout is real. When you struggle, it’s not only extremely hard on you, but it also is difficult for the person you’re helping. Fortunately, ElderSource has a program designed to prevent caregiver burnout.

The Tailored Caregiver Assessment and Referral  Program (TCARE) is an accredited program that offers customized support to family caregivers.

If you are eligible if you are a caregiver supporting a loved one who is age 60 or older and resides in Baker, Clay, Duval, Flagler, St. Johns, Nassau or Volusia County.

 

What TCARE Does:

  • Reduces your stress and provides strategies for reducing root causes of caregiver burnout
  • Creates a care plan just for you that offers support and improves your effectiveness as a caregiver
  • Connects you with resources that best meets your needs
  • Promotes aging in place and may delay nursing home placement for your loved one

How TCARE Works:

ElderSource’s TCARE specialist meets with you twice over the phone. In the first meeting, the specialist learns your sources of stress and individual strengths. Then a care plan is created for you and discussed in the second meeting. The specialist reaches out every three months to check on you. * All meetings are and scheduled Monday through Friday, 9 a.m. to 5 p.m.

 The TCARE program has been made possible by a generous grant from the Florida Blue Foundation. The multi-year grant is designed to support organizations in helping their clients overcome additional challenges caused by the COVID-19 pandemic. AHEC and Aging True are grant partners that will support the program by providing additional direct mental health services to older adults and caregivers as determined by the TCARE assessment.
